SMS/MMS Mobile Messaging Program Terms

1. You will receive messages related to your scheduled appointments, reminders, cancellation notices, and future marketing promotions or specials we are offering from Madison Wellness & Aesthetic Center.
2. You can cancel the SMS service at any time. Simply text “STOP” to our number or shortcode. Upon sending “STOP,” we will confirm your unsubscribe status via SMS. Following this confirmation, you will no longer receive SMS messages from us. To rejoin, sign up as you did initially, and we will resume sending SMS messages to you.
3. If you experience issues with the messaging program, reply with the keyword HELP for more assistance, or reach out directly to [email protected].
4. Carriers are not liable for delayed or undelivered messages.
5. As always, message and data rates may apply for messages sent to you from us and to us from you. You will receive less than 3 messages per month. For questions about your text plan or data plan, contact your wireless provider.

Eligibility & Promotional Offers

This offer is valid for new patients only and may be redeemed for the specified service listed in the promotion. By participating in this program or redeeming this offer, you represent and warrant that you are at least 18 years of age. Offer Expiration

Offers must be redeemed within 30 days of form submission. Appointments must be scheduled and completed within this time frame to receive the promotional rate.

Limitations

• Offer has no cash value and cannot be exchanged for other services.

Scheduling

Appointment availability is limited and based on provider availability. Submitting a form on this website does not guarantee an appointment time or availability.

Cancellation Policy

Cancellations or reschedules must follow the practice’s standard policy (e.g., 24-48 hours’ notice). Failure to comply may result in forfeiture of the offer.

Right to Modify

Consultation Requirement
